To be approved for an intensive treatment program, every patient submits a Health Information Form. Download it below, then call us before sending any medical information.
Health Information Form
Required for all intensive treatment programs, including Intensive Suit Therapy. The form is a four-page PDF you can print or fill in and return to us.
Records to include with your form
- MRIs and/or all x-rays related to the diagnoses being treated
- For children with possible hip subluxation or dislocation: the most recent written report and a hip x-ray taken within the past six months
- Please contact us before faxing any completed forms, x-rays or other medical information
